Student Health Requirements
As Stated by Iowa Department of Public Health
Preschool:
- Current Health Physical (within the last year)
- Immunizations up-to-date Iowa Immunization Laws and You
Kindergarten:
- Immunizations up-to-date
- Dental Screen (No earlier than age 3 and no later than 4 months after enrollment)
- Vision Screen (No earlier than 1 year prior and no later than 6 months after enrollment)
- Lead Test (At least one test prior to 6 years of age)
- Health Physical (Recommended)
Third Grade:
- Vision Screen (No earlier than 1 year prior and no later than 6 months after enrollment)
Seventh Grade:
- Tdap and Meningococcal Vaccine #1 (see above link for immunization schedule)
Ninth Grade:
- Dental Screen (No earlier than 1 year prior to enrollment and no later than 4 months after enrollment)
Twelfth Grade:
- Meningococcal Vaccine (Dose #2)

Classroom snacks and celebrations
Please be mindful of nut allergies in our school district. Food items containing nuts or that have been processed in a facility where cross contamination may occur are not allowed. We request that celebration treats are individually packaged with the food ingredients attached. No homemade products will be allowed.
